FAIRLAWN – The Village of St. Edward partnered with Summa Health System for enhanced health care services and with Senior Helpers as a preferred provider for supportive services to residents on its campus.

“This unique partnership serves as a model of care for delivering health services,” said Kim Serra, business development coordinator for Summa Homecare and Hospice Services.

“Our goal is to have our residents age in place,” said Annette Walters, director of marketing and admissions at The Village of St. Edward. “If we see a resident who lives in Independent Living and they are struggling with daily activities such as managing medications, Senior Helpers can step in and provide medication management. If we observe an Independent Living resident who may need some additional assistance with personal care, Senior Helpers can be there to assist.”

According to St. Edward officials, the Summa team will be led by Dr. Nancy Istenes as medical director with onsite physician Dr. Dupal Patel. Patel’s focus will be on Independent Living and Assisted Living residents and she will have scheduled clinic hours. Summa at Home staff will provide post-acute services including home care, hospice, palliative care and home infusion, according to St. Edward officials. Additionally, Summa also will provide programs for mental health nursing, telehealth monitoring to support disease management and other services tailored to meet needs of residents. Summa at Home staff will include board-certified physicians, nurse practitioners, registered nurses, therapists, social workers and home health aides.

“We are very excited about this unique partnership,” said Walters. “Our mission is to provide the optimum level of care for all residents throughout our campus. Our partnership with Summa is not exclusive. Our residents will continue to have options and choices when it comes to their care. We are always looking for ways to improve our resident’s services. This provides a convenient option for them.”
